Parents/guardians of all children being considered for enrollment at the Kiwanis Day Care Center must present the following:
- Completed Physical Examination (less than one year old)
- Current Immunization Record
- Copy of Health Insurance Coverage with Policy Number
- Completed Center Application Packet (List Below)
- Payment of all fees required
- Change of clothing
- Formula/food (where required)
- Diapers (where required)
- Special Dietary Needs Form (where required)
